A Deconstruction Approach to Therapeutics

MODERN MANUAL THERAPY

Today’s surplus of information seldom solves even moderately complex pain patterns. The solutions frequently reside in the ability to listen, with our ears and our hands. Listening with our hands refers to utilizing them as perceptive organs, rather than vehicles to create and extend our plans.

The locomotor era of physical rehabilitation has peaked. Of course we’ll see new limits eke out as we do with physical therapies siblings, sports and conditioning.

Movement will always have prominent role in physical rehabilitation, analogous to a musician practicing their cords, movement is critical to understanding the current capacity of the instrument. But the idea that physical conditioning is a comprehensive solution for chronic pain sufferers simply fails to deliver.

Current perspectives on pain testify that pain is sensorial and subjective.

And in many cases, external measures are not reliable indicators of why someone is experiencing pain. This raises the bar for our therapeutic interactions. Assessments become more nuanced, strategies more refined, and conversations clear and empathetic.

Improved movement signals sensorial and qualitative information to the subjective/experiential self, which then updates its pain threshold to more accurately represent it’s proprioceptive, interoceptive and visual feedback. Perceiving is believing.

A few decades before physical therapist's were ushered onto the allopathic playing field to treat polio victims and injured war veterans. The field of Osteopathy had made its debut. At its inception, osteopathy was a hands on healing art. The core tenet being that the health of the individual could be restored by recalibrating key areas that were separated or disassociated from their inherent health. In the1960’s the American branch of osteopathy was politically acquired by the American Medical Association. Osteopathies flickering core tenet was left for others to tend.

To paraphrase its founder: Osteopathic principles are as old as the universe, understanding and putting them to use is a human birthright.

A.T. Still had extraordinary vision, he knew osteopathy was an iteration of a timeless principle.

Specific Areas of Focus

The tissue specific approaches of Myofascial Release and Neural Tissue Tension.

Cranial Therapies expanded the range of people that may be helped. Concussions, whiplash, spinal and cranial surgeries, as well as cranial facial pain patterns secondary to dental procedures, TMJ and headache patterns were understood in fresh light.

Visceral Manipulation opened access to the three dimensional body. The cavities and their contents are frequently the crux of a persons pain patterns. Releasing the tension of post surgical scars and their subsequent effects often amazes the patient, and frankly it still amazes me. The ability to balance pressures and tensions skewed by falls, whiplash, and athletic contact were put in my hands. I am thankful!

Techniques are important, but integration of the various modalities and perspectives of the body afford the client a real advantage in finding the results they seek. Find a therapist that has the tools and knows how to use them.

About Joseph Shindoll

Joseph is an educator, author, practitioner and founder of Soma Solutions.

He's a licensed unified manual therapist with over 30 years experience in the field.  Joseph is a member of International Alliance of Healthcare Professionals.

Joseph has studied many osteopathic-based approaches to manual therapy.  The mechanical approaches of Strain Counterstain, Muscle Energy, and Lauren Berry work.

  • Graduated New York College of Health Professionals 1992

  • Worked as a Holistic Health Practitioner San Diego, Ca 1993

  • Participated in hospital based massage research University Hospital, Denver Co 1994

  • Began first year cycle Lauren Berry training Berkley, Ca 1994

  • Continued Lauren Berry program 1995

  • Began studying Muscle Energy Technique and Strain CounterStrain 1995-96

  • Began Upledger Cranial Training 1996 while working at a busy Headache and cervical rehabilitation clinic in Denver 1995-98

  • Began Barral’s Visceral Manipulation program 1997

  • Therapeutics Horizons comes on the scene; a joint teaching venture of Frank Lowen, the head of Barral’s VM program in the United States at the time, and Sharon Weiselfish Giamatteo who headed up the integrative programs MET, MFR, SCS, etc. at Upledger. Sharon would go on to extrapolate and systematize these methods creating Integrative Manual Therapy.  Therapeutic Horizons generated a vast array of information and techniques. They preempted the discovery of the glymphatic system and were emphasizing attention to venous and lymphatic drainage prior to cranial work prior to the turn of the century. The neuro-vascular relationship and the importance of vascular integrity for neurogenesis was  emphasized this was not a common concept at the time.

  • Systems thinking brought clinical reasoning to new levels and treatment plans were structured accordingly. Typical patterns of somatic response were identified then used to assess and treat.

  • Joseph studied and worked with Frank and Sharon from 1998-2002

  • Joseph continued studying and working with Frank Lowen through 2005 as he developed Biovalent Systems which focused on regulatory patterns I.e. points and patterns that regulated aspects of a tissue or functional system. E.g. Biomechanical regulators, Fascial fulcrums, and Manual perceptual maps. Frank continued his research and practice developing Dynamic Manual Interface. Which focused on pressure regulation of the neural, hormonal, and vascular systems.
